Going on holiday this summer? Needing travel vaccinations? Here’s a couple of things to remember:

  • You need to fill out a Travel Risk Assessment form which can be given to you at reception OR download here: travel risk assessment form.pdf (once you have downloaded the form, you need to bring it to reception as soon as possible.)
  • Our nurses needs at least 6 – 8 weeks notice before your holiday date so we can see what vaccines you have already have and to find out which ones you will need.
  • There is a charge for some vaccines. The nurse will inform you of this before your appointment and you must bring your money on the day.

If it is deemed appropriate , you may be offered an appointment at one of six GP access centres across Sheffield 6pm-10pm weekdays and 10am-6pm weekend.

The Crookes Practice  6pm-10pm Mon/Fri , 10am-6pm Sat, 10am-2pm Sunday.

Woodhouse Health Centre  6pm-10pm Mon/Fri, 10am-6pm Sat, 10am-2pm Sunday.

We are proud to be involved in training medical students and doctors at the practice.  Dr Maher is a trainer for doctors training to be GPs. Our current registrar is Dr Kath Kneen.  There are second year medical students attached to the practice throughout the year.

The surgery has access and facilities for people with disabilities and there is free parking available at the rear of the building. There is a hearing loop installed.  Please speak to reception if you require help.
